AO3 doesn't record all hits -- for example I just looked at the page twice, and it still shows as having 31 hits :-p I suspect the hit counter is JavaScript-driven and if you have JS disabled then it doesn't register....
But I'm pretty sure there *is* a bug which can cause large amounts of non-existent guest kudos to appear, because the same thing happened on one of my own fics: I received 19 kudos from 11 page hits, and then another 8 kudos after only 1 more page hit. In fact those 27 highly suspect guest kudos points still represent a large proportion of the total kudos on that story... ("There is definitely something weird going on with my AO3 stats...")
